Patient registries, whether free standing or tied to regional Health Information Exchanges, are fundamental to the development of the "medical home" patient and provider relationship.  Registries have been demonstrated to significantly improve key quality measures in the provision of preventive services and treatment of several chronic conditions including diabetes, hypertension and asthma.  Registry information is used to track the needs of individuals as well as developing more effective approaches to managing the health needs of the population.
