 Getting Patients Into the Clinic Process

 
 

VODI is an organized system of care that enrolls, manages, and tracks the care delivered to a Detroit's uninsured residents. These VODI members qualify for the program under the following eligibility requirements: 

  • are 18-64 years of age
  • live in Wayne County 
  • have incomes that fall below 200% of the federal poverty guidelines

VODI provides our members with a primary care "medical home" within one of the partnering members. At that "medical home", VODI members receive primary care, including regular and routine health exams, screening tests, preventive services, health education and care management.

Individuals can contact a clinic near their residence, from our participants lists above, to make an appointment.  They will need to bring identification, and proof of income to be enrolled.
